Definition and Epidemiology

Etiology

Antenatal Genetic syndromes (Fragile X, Rett syndrome, Prader-Willi), Chromosomal disorders (Down syndrome, Klinefelter syndrome, 1p36 deletion), Cortical malformations (lissencephaly), Intrauterine infections (syphilis, toxoplasmosis, rubella, cytomegalovirus, Zika virus), Teratogen exposure (fetal alcohol syndrome), Inborn errors of metabolism.
Perinatal & Neonatal Hypoxic ischemic encephalopathy, Kernicterus, Hypoglycemic brain injury, Hypothyroidism, Neonatal meningitis or encephalitis, Intraventricular hemorrhage, Periventricular leukomalacia.
Postnatal Deficiency of vitamin B12, iodine, or B1; Toxins (lead exposure); Traumatic head injury; Postnatal central nervous system infections; Severe nutritional deficiencies.

Clinical Features and Red Flags

Newborn Dysmorphic syndromes, multiple congenital anomalies, microcephaly, major organ system dysfunction (e.g., feeding, breathing).
Early infancy (2-4 months) Failure to interact with the environment, concerns about vision and hearing impairments, unusual muscle tone (hypotonia or hypertonia).
Later infancy (6-18 months) Gross motor delay (lack of sitting, crawling, or walking).
Toddlers (2-3 years) Language delays or significant communication difficulties.
Preschool (3-5 years) Language difficulties, behavior difficulties, delays in fine motor skills (cutting, coloring, drawing).
School age (>5 years) Academic underachievement, behavior difficulties (attention, anxiety, mood, conduct).
